Just as a physician reviews a patient's risk for heart disease by assessing diet, exercise habits, and blood pressure, we now have the ability to review lifestyle habits, dental hygiene, and other risk factors for cavities our patients may present with. This simple, one-minute questionnaire covers disease indicators such as radiographic and white spot lesions along with such risk factors as diet, dental hygiene, saliva flow, medications, bacterial population, and dental history to identify risk for decay.
This risk assessment form is a simplified version of the form developed by Dr. John Featherstone through his extensive caries research at University of California, San Fancisco. It has been modified with his approval and the aid of Dr. V. Kim Kutsch, Dr. Doug Young, and Dr. John C. Kois. to create a form that is quick and easy for use in private practice.
